Abstract:

Objective: Classical anatomical texts describe the spinal dura mater as a free-floating structure within the vertebral canal. Earlier writings, however, have described dura mater attachments to the spine. The purpose of this study is to review the descriptions found in the literature and compare them with an experimental study. Methods: The anatomical study was conducted on three female subjects with a mean age of 84 years and on a female fetus estimated to be approximately 28 weeks gestational age. A histological study using serial sections was also performed on a male subject (93 years old). Results: Held in place by its attachments at the foramen magnum and by the filum terminale, the dura mater is attached laterally to the bony walls by Forestier’s opercula and Hofmann’s ligaments. Forestier’s opercula are drum-skin-like structures that line the openings of the intervertebral foramina. Hofmann’s ligaments develop between the anterior surface of the spinal roots and the posterior longitudinal ligament (PLL); their orientation varies depending on the vertebral level. Anteriorly, the dura mater is held in place by Trolard’s ligament, a median sagittal structure composed of arched fibers, though it may also appear as a complete septum or as a strong adhesion between the dura mater and the PLL. Conclusion: The spinal dura mater is therefore not entirely devoid of attachments to the spine. These insertions play a role in the dura mater’s adaptive response to changes in the size of the vertebral canal. They also serve to modulate, restrain, and control mobility and growth. These spinal insertions are also directly connected to the neurovascular bundle at the foramen magnum. With each flexion or extension movement of the spine, these attachments tighten or relax and play a genuine role in vascular pumping, enabling optimal circulation between the intra- and extra-spinal vascular networks. Abstract original language:
