Tension-type headache or sinister masquerader?

Abstract:

This paper discusses a case of headache in an otherwise healthy 33-year-old male, which at first appeared to be tension-type headache. The initial clinical picture was slightly complicated by co-existing mechanical dysfunction of the thoracic spine. In the absence of objective cervical dysfunction, and in light of progressively worsening headache symptoms the patient was referred for further investigation. Subsequent imaging identified an intracranial space-occupying lesion, which was successfully removed at surgery. The case highfights the importance of identifying clinical findings that may suggest underlying pathology as a cause of headache. The International Headache Society criteria for diagnosis of tension-type headache, and associated clinical features are also presented.
