Effect of acupuncture combined with rehabilitation on Postoperative gastrointestinal dysfunction after abdominal operation

Abstract:

Background: Postoperative gastrointestinal dysfunction is one of the most common complications in patients undergoing abdominal general anesthesia. The incidence of PGD in patients undergoing abdominal surgery is 10% to 30%. PGD patients have a long hospital stay, heavy economic burden, high risk of infection and other complications. Maintaining normal gastrointestinal function is considered an important part of ERAS after abdominal surgery, reducing the pain of surgery, preventing organ dysfunction and complications, and speeding recovery. At present, the treatment of PGD mainly focuses on multi-modal approaches, including drug intervention and other non-drug strategies. Drug interventions may have potential side effects and high costs, which may reduce acceptability. Acupuncture and rehabilitation therapy have gradually become an important supplement to the comprehensive prevention and treatment strategy of PGD with the advantages of multiple targets, exact effect and non-toxic side effects. The purpose of this study was to explore the efficacy of acupuncture and rehabilitation for PGD and provide research data. Methods: Sixty patients undergoing abdominal surgery (hepatobiliary and pancreatic related surgery) in the First Hospital of Lanzhou University were divided into 3 groups, including drug therapy group, acupuncture+press-needle therapy group and acupuncture+press-needle+ comprehensive rehabilitation therapy group (visceral manipulation+physical factor therapy). Among them acupuncture therapy used the acupoints (Hegu, Neiguan, Quchi, Zusanli, Sanyinjiao, Shangjuxu, Xiajuxu, Zhongwan) to Deqi and the needles were retained for 20 minutes. Press-needle therapy (Dachang point and ear points, etc.) retained needles for 2 days, and press 20 times at each point every 2 hours. On the 1st, 2nd and 3rd day after operation, acupuncture+press-needle, acupuncture+press-needle+rehabilitation comprehensive treatment were operated. Acupuncture treatment lasted for 30 minutes until the patient's exhaust recovered, and then comprehensive rehabilitation treatment was performed. The main outcome measures were first bowel movement time, and the secondary outcome measures were first exhaust gas, time to get out of bed, perioperative SP concentration change, the incidence of PGD, postoperative pain and postoperative nausea and vomiting to determine the efficacy of acupuncture combined with rehabilitation for postoperative gastrointestinal dysfunction. Results: The first bowel movement time, the first exhaust gas, time to get out of bed, perioperative SP concentration change, the incidence of PGD, postoperative pain and postoperative nausea and vomiting to determine the efficacy of acupuncture combined with rehabilitation for postoperative gastrointestinal dysfunction. Conclusions: Acupuncture combined with rehabilitation has a good clinical effect on postoperative gastrointestinal dysfunction.
