Abstract:

The study has a quasi-experimental design in the parameterd of a non-randomized and non-blind study. There is a continous devision into the respektive groups. The first five subjekts are placed into the interventation group, the next five into the control group, and so on..21 subjekts took part of the study.10 in the control group and 11 in the interventation group. The dependent variables for all subjekts are two pure tone audiograms and two questionnaire. For the interventation group the independent varables are the medicinal treatment and the osteopathic treatment. For the control group the independent variable is the medicinal treatment. Outline/Problem Definition The selection of the topic „hearing loss“ was based upon my experiences in the osteopathic practice. My interests were directed to this disease pattern through the treatment of patients with an acute reduction of hearing. During my research about osteopathic treatment models for an acute hearing loss, I could only find two osteopathic publications: a DO-thesis from Cöln (2000) and a master thesis from Staffa (2006) at the Donau Universität Krems. There is plenty of alleopathic research on this topic. So far an obvious cause for hearing loss could not be found, and therefore a uniform treatment concept is missing (guideline hearing loss, 2004). Research Question & Objective In the masterthesis, the limitation to hearing loss in the low frequencies was done, since according to current research these disturbences are more likely to be influenced by manual medicine than disturbances in high frequences. Wagner and Fend (1998) report of hearing loss in low frequencies that was successfully treated with a manipulation of the cervical spine. Hülse et al. (2005) see a connection between a low frequency hearing loss and dysfunctions of the upper cervical spine and names this coherence a „vertabragenic hearing disorder“. Hypothesis This study serves as a verification of my hypothesis, that an early osteopathic treatment of patients with an acute hearing loss in the low frequencies, parallel to the medicinal treatment, has a stronger effect on hearing, than a sole medicinal therapy Relevance for the Patients The patients with an acut hearing loss use to go to the ENT-physicians very fast. Normally they get different medicinal treatment. In this study, it looks like, that ostopathy could be a good alternativ in the acute phase. The patients have do get information about osteopathy as an therapyoption from their ENT-physicians or directly from us. Relevance for Osteopathy The aim of my study was to examine the effect of an osteopathic treatment for an acute hearing loss in the low frequencies. In my literature study I could not find according osteopathic research, that was done prospectively, randomised and controlled. Methodology This study has a quasi-experimental design in the parameters of a non-randomized and non-blind study. Five subjects were alternately placed in the intervention group (osteopathic and medicinal treatment) and the control group (medicinal treatment). 21 subjects took part in this study. Dependent variables are the pure tone audiograms and the questionnaire. The independent variables are the two osteopathic treatments according to the general osteopathic principles and the medicinal therapy according to the guidelines of the deutsche Gesellschaft für Hals-Nasen-Ohren-Heilkunde, Kopf und Hals-Chirurgie (German Association for Ear, Nose and Throat medicine, head and neck Surgery). I do not know of a standardized questionnaire in reference to hearing loss. Therefore, special questionnaires are devised for this study. The questions refer to the subjective physical and psychological condition of the subject. The subjects receive two questionnaires. One is used to find out if there are additional symptoms that occur parallel to the low frequency hearing loss. And the second questionnaire, containing the same questions as the first questionnaire, clarifies whether a change takes place in the six weeks. In the intervention group the first questionnaire is filled out before the osteopathic treatment. The second questionnaire is filled out after six weeks. In the control group it is done at a parallel time. The data was analysed in SPSS (Version 17). Results The study shows a significant improvement of hearing after six weeks in the interventation group aswell as in the control group. However the increase of hearing in all four frequences (0.125-0.75kHz) is significantly higher (p=<0.05) in the interventation group compared to the control group. The accompanying symptoms of vertogo, Critical Reflection/Perspectives/Conclusions This study indicates that an osteopathic treatment for an acute hearing loss in the lower frequences has a positive effect on the hearing. Therefor, osteopathic diagnosis with an according treatment should be considered as a therapy option.
