Abstract:

Traction Alopecia (TA) is a common yet preventable hair loss condition caused by prolonged or repetitive mechanical tension on the hair follicle. Although initially reversible during its early inflammatory stage, delayed recognition may result in permanent scarring alopecia. Hair plays a distinctive and important role in personal identity, closely tied to self-esteem, confidence, and social identity highlighting the need for patient-centered management. While previous reviews have focused primarily on the dermatologic features of TA, this review applies an osteopathic whole-person framework that recognizes the interconnected biologic, behavioral, cultural, occupational, psychosocial, and environmental factors contributing to disease development, progression, treatment adherence, and prevention. We synthesize current evidence regarding epidemiology, risk factors, clinical presentation, trichoscopic and histopathologic findings, diagnosis, and conventional management. Options to mitigate the progression of or treat TA include behavioral modifications, pharmacological interventions, and osteopathic manipulative treatment (OMT) alongside other adjunctive modalities, such as mesotherapy, nutraceuticals, and alternative medicine. Effective management extends beyond restoring hair growth and instead requires a holistic integrated approach that combines evidence-based dermatologic care with culturally responsive education, behavioral modifications, psychosocial support, and appropriate adjunctive therapies. Future research should further evaluate osteopathic interventions and other complementary treatment strategies to expand the evidence-base supporting comprehensive TA management.
