Abstract:

Study Design Methodological-clinical study. Outline/Problem Definition Any sort of mutual consent in manual diagnosis have a rare and poor reliability.Manual diagnosis is a tool for many osteopaths in their analytical methods and findings.It is the requirement of an osteopathic treatment and focuses on a holistic screening of the patient.One of the chief aspects in osteopathic diagnosis is the evaluation of the visceral system. The diagnostic interpretation of the tension of the abdomen is on of the first step for continuing diagnostic( according to the concept of J.Weischenk(1982) and L. Fieuw(2003)). The aim of my study is to show if the results of a manual palpation of both tension and tonus of the abdominal can generally be judged as equal on a common basis. In the past years , scientific research and investigating in palpatory diagnosis has been extremely rare , which makes the output of this study even more important Research Question & Objective Can a reliable statement be made about the tension about the tension of the viscera and the tonus of the abdominal wall in a standing position with several different osteopaths palpating the hypochondral – umbilical- and inguinal region of the abdomen(According to the anatomical concept of F.Glenard (1905) and descibed by J.Weischenk (Traité d`osteopathie viscérale(1982)) as an manual examination) Hypothesis Accordimg to most inter- and intrarater diagnostic studies,the reliability of suchlike surveys is unpromising and can only be interpreted as an subjektive diagnostic tool that can at best only improve further diagnostic steps but not as an objective overall diagnostic instrument Relevance for the Patients If manual diagnostics of the abdominal tension and the tonus of the abdominal wall offers a scientific results it is an extremly suited tool for further diagnostic clarification of the patient. Relevance for Osteopathy Osteopathical therapy qualifies for a holistic treatment of the patient. In the 80th the french osteopaths( J.Weischenk , J.P.Baral ) integrated the visceral system into the holistic osteopathic concept. There are a lot of diagnostics manual tests to appoint the visceral function. According to the concept of J.Weischenk(Traité d`osteopathie viscerale,1982) ,L.Fieuw(Osteopathische Techniken im visceralen Bereich,2005) and J.Helsmoortel(Lehrbuch der visceralen Osteopathie,2002) affects the tension and mobility of the content(intestinum,visceral organs) the function/dysfunction of the cover(skeleton,skeletal muscles,…). To measure the function and mobility of the visceral system the manual palpation of the tension of the content and the tonus of the cover in a standing position is one of the basic tests for further diagnistic examination. It is my aim to prove the reliability of this test and its information value for the manual examination in osteopathic diagnostic Methodology The basis of this survey is an inter- and intra-reliability design with repeated measurements to be able to testify the efficency of assessing the tone and tension of the abdomen. Before the examination, all osteopaths were trained with the objective of achieving a coordinated test-performance and interpretation. In the subsequent examination, seven osteopaths twice analyzed 33 patients. In the second run, the osteopaths carried out the examination wearing sleeping masks to prevent them from optically recognizing their patients. To assess tone and tension, the criteria hyper/normo/hypo were introduced and documented in a specific examination form. Additional assistents were appointed to document the findings and to ensure a smooth test run. Results In general, the results of an assessment of the tone are more reliable (moderate reliability) than an evaluation of the abdominal tension (poor reliability). In total, it must be stated that the reliability of this test is too low. Critical Reflection/Perspectives/Conclusions Based on the results of this survey, a general trend in manual medicine can be confirmed. This trend points to the fact that in terms of an average rating, the findings of intereximiner reliability are less reliable than those of intraeximiner reliability. A final assessment of this examination shows that the scientific investigation of both the “tension model of Glénard“ as well as its use in visceral osteopathy is insufficient. The present study is criticizing this situation and would like to suggest a further exploration of this revealing aspect of visceral osteopathy.
