Abstract:

Study Design: Quantitative social research. Outline/Problem: Definition In orthodox medicine, scoliosis patients who are still growing are subjected to continuous and regular clinical follow-ups by orthopaedic surgeons. During this time, some patients also receive physiotherapy. This includes the order to deliberately build up muscles by means of exercises. Thanks to physiotherapy, there are already well-developed specific training and therapy concepts such as the Katharina Schroth method (Lehnert-Schroth 2000). With greater angles of curvature, the exercise method according to Katharina Schroth is recommended (Orthopaedic Hospital Vienna-Speising 2008). With further progression, for example with a Cobb angle of between 20 ? 45°, orthopaedic surgeons prescribe a brace that has to be worn up to 23 hours a day over a period of several years until the bones have completely stopped growing(Wirth und Zichner 2004, Hüter-Becker and Dölken 2005). It is intended to prevent patients from suffering further curvature and consequently a spinal fusion operation, which is an option that is mooted when curvature reaches a Cobb angle of 45° or more (Weiß 2000, 2003). As osteopathy is a holistic therapeutic approach, the method of care described above is quite capable of further development and its quality can be improved by means of osteopathic measures. In osteopathy, scoliosis patients receive individualised treatment, depending on their age, sex, general state of health, manifestation of curvature and lesion patterns. The approach is very heterogeneous, not only because of the holistic nature of osteopathy but also, possibly, because of the complex clinical picture. Research Question & Objective How do Austrian osteopaths assess their therapeutic experience with the clinical picture of idiopathic scoliosis? Hypothesis 1. Osteopaths during there education gained little specialist knowledge about idiopathic scoliosis and the concerning treatment options. They use different sources to improve their specialist knowledge. 2. Therapists, who use Osteopathy as a treatment method for idiopathic scoliosis, show a heterogeneous picture regarding their approach. 3. Osteopaths deem the osteopathic scoliosis treatment as an important and effective part of the rehabilitation process. They partly achieve good therapeutic success. However during their treatments they still see unclarities. Methodology With reference to the research question a one-off, written survey using a standardised questionnaire was chosen as the measurement method for this study. The questions drawn up by me were divided into three thematic groups. In the course of a random description, additional questions regarding the personal details of the participants were asked at the beginning. The thematic areas comprised their level of knowledge, how they derived their specialist knowledge as well as the general assessment of the various components of the osteopathic treatment process. Assessment of the general treatment options and the effectiveness of osteopathic treatments with regard to idiopathic scoliosis as well as future possible improvements represented a further area. The professional control of the contents were also done by 2 other osteopaths and the technical construction was supported by a sociologist an a market research department of a business company. 300 Osteopaths were invited to answer the questionnaire. Results The responding rate was 42 % of Osteopath who at least started to answer the questionnaire. Those Osteopaths regard osteopathy as an important therapeutic method for scoliosis patients ? very often, as equally important as physiotherapy (79%). They also believe that osteopathic treatments are important especially with regard to early detection (99%). During their education, Osteopaths didn't achieve much knowledge about scoliosis (86%) ? they use other resources to improve in this field like literature (49%) and communication with experienced colleagues (33%). Despite the low specialist knowledge of scoliosis gained during their entire professional training period, a major part of the participants deemed their skills in treating scoliosis patients to be Critical Reflection/Perspectives/Conclusions I didn't find out, if specialist knowledge is essentially important for osteopaths in treating patients with idiopathic scoliosis.Furthermore I can not say that there is a lake of teaching specialist knowledge during the education of osteopathy because I didn?t research if this is required for osteopathic students. Perspectives and Conclusion: Orthodox medical literature also points again and again to the individual nature of idiopathic scoliosis, its early diagnosis as well as to expedient and timely treatment. Thanks to its extensive repertoire of treatment options, osteopathy offers precisely those components that are lacking in current and established conservative therapies.
