Osteopathic techniques and manual rotation of the fetal head successfully resolve mechanical dystocia

Abstract:

We read a paper The impact of femoral rotation on sacroiliac articulation during pregnancy. Is there evidence to support Farabeuf's hypothesis by finite element modelization? with a great interest [ [1] ]. We are referring to it from a clinical perspective. In our experience, sacroiliac (SI) articulation plays an essential role in the mechanics of labor. In the period from the 27th of April 2021 to the 19th of May 2022 in Clinical Hospital Sveti Duh, an obstetrics and gynecology resident and manual therapist applied osteopathic techniques and manual rotation of the fetal head (MR) in term labor to resolve mechanical and reduce the number of cesarean sections. Osteopathy is a form of complementary and manual medicine that uses the therapeutic application of manually guided forces to improve physiologic function and support homeostasis altered by somatic dysfunction. It uses various techniques to increase the range of motion, improve tissue texture, decrease pain, and regain symmetry [ [2] ]. Twelve women were treated in the second stage of labor because of the arrest of fetal head engagement. Contractions were already adequately stimulated with oxytocin, and MR was performed unsuccessfully. At the time of treatment, the criteria for cesarean or instrumental delivery were not yet satisfied. Instead of expectant management, detailed obstetric status was obtained following the postulates of classical obstetrics and osteopathy. Population, findings, indications, treatment and outcomes are presented in Table 1.
