Abstract:

Introduction: Heart defects are the most common congenital anomalies worldwide. Over the past decade, improvements in prenatal screening and early management have led to a better prognosis for these infants with heart defects. If a heart condition requires surgery, it is generally performed during the first year of life. This surgery results in a sternal or thoracic scar that can cause significant postoperative pain. The objective of this study was to determine whether osteopathic care could facilitate recovery in newborns and infants by reducing pain. Methods: Eighteen patients were randomized into two groups: a treatment group (n = 10) and a control group in which osteopathic care was simulated (n = 8). The study was conducted in the pediatric cardiology department at Necker Hospital within the first three weeks after surgery. The objective assessment criteria included the EVENDOL scale, an osteopathic score called OSCAR, and cardiac parameters, which were recorded on the day of the consultation and one day later. Results: A significant decrease was observed in the EVENDOL scale score as reported by parents (p = 0.01) and the child’s treating physician (p = 0.03), as well as in the OSCAR osteopathic score, in the treatment group. Regarding vital signs, there was a significant decrease in respiratory rate (p = 0.04) in the treatment group. Conclusion: Osteopathic treatment focused on the consequences of cardiac surgery reduced postoperative pain in the participants of this study. Introduction: Les cardiopathies sont les anomalies congénitales les plus répandues dans le monde. Depuis une dizaine d'années, l'amélioration du dépistage anténatal et de leur prise en charge précoce a permis un meilleur pronostic vital pour ces nourrissons cardiaques. Si la cardiopathie nécessite une chirurgie, elle intervient généralement pendant la première année de vie. Cette opération laisse place à une cicatrice sternale ou thoracique pouvant entraîner d'importantes douleurs postopératoires. L'objectif de cette étude était de savoir si une prise en charge ostéopathique permettrait de faciliter la récupération des nouveau-nés et nourrissons en diminuant la douleur. Méthodes: 18 patients ont été randomisés en deux groupes : traité (n = 10) et contrôle où la prise en charge ostéopathique était simulée (n = 8). L'étude a été réalisée au sein du service de cardiologie pédiatrique de l'hôpital Necker dans les trois semaines postopératoires. Les critères d'objectivation regroupaient l'échelle EVENDOL, un score ostéopathique nommé OSCAR et les constantes cardiaques qui ont été notées le jour de la consultation et un jour après. Résultats: Diminution significative du score de l'échelle EVENDOL remplie par les parents (p = 0,01) et le médecin suivant l'enfant (p = 0,03) et du score ostéopathique OSCAR dans le groupe traité. Pour les constantes cardiaques, importante diminution de la fréquence respiratoire (p = 0,04) dans le groupe traité. Conclusion: Un traitement ostéopathique orienté sur les conséquences d'une chirurgie car-diaque a diminué les douleurs postopératoires des participants de cette étude. L'ostéopathie pourrait être intégrée au sein d'une prise en charge pluridisciplinaire dans les suites opératoires d'une chirurgie cardiaque chez les nouveau-nés et nourrissons.
