The effectiveness of osteopathic intervention on the diaphragm in intensive care unit patients on long-term mechanical ventilation

Abstract:

Introduction. The long stay of patients in intensive care units (ICU), especially requiring prolonged artificial lung ventilation (ALV), is one of the most acute problems of modern critical condition medicine. The pursuit of early excretion from respirator and restoration of self-adequate breathing is a key objective of intensive therapy, as the reduction in duration of ALV directly correlates with improved prognosis, lower frequency of complications, Reducing hospitalization and improving the quality of life for survivors. However, conventional rehabilitation methods involving respiratory gymnastics and verticalization are often insufficient for full recovery of respiratory function in patients with pronounced biomechanical disorders due to long-term immobilization. The medicamentous sedation and pathological processes leading to hospitalization require a comprehensive approach. Despite increasing interest in integrative treatment methods, the impact of osteopathic correction (OC) on gas-exchange dynamics and duration of ventilation support in critical long-term ALV patients remains poorly understood. The aim of the study: evaluate the effectiveness of including osteopathic correction in complex therapy for long-term ALV patients in the intensive care unit. Materials and methods. The randomized controlled study was carried out in the department of intensive care and intensive therapy 2 GKB named after S. S. Yudin (Moskow). The study included 48 patients who met the following inclusion criteria: age over 18 years, clear consciousness, productive contact, adequate analgesia, remaining on full respiratory arrest for more than 5 days. All patients were treated and examined according to their underlying disease and complications, as prescribed by the attending physician, including setting respiratory support regimens, performing therapeutic gymnastics, verticalization, and speech therapy if necessary. Patients were randomly divided into two groups: the control group included 24 patients who were not treated with OK; the main group included 24 patients who were treated with OC. To objectify the patient′s condition and assess the effectiveness of treatment, standard scales and clinical indices for patients were used ICU: Glasgow Coma Scale (GCS) — consciousness level assessment scale, Sequential Organ Failure Assessment (SOFA) — a scale for assessing the degree of organ dysfunction, Richmond Agitation-Sedation Scale (RASS) — a scale for assessing the level of sedation and excitation, oxygenation index (PaO2 /FiO2, p/f) — an indicator of the effi ciency of gas exchange in lungs. An osteopathic diagnosis of the patient within the bed was also performed, evaluating the function of the diaphragm and the region of the thorax using the method proposed by Bruno Bordoni (2016). Results. All patients were found to have «inhaled» diaphragm dysfunction. The inclusion of OC in the set of therapeutic interventions for ICU patients resulted in a statistically significant reduction in the number of days spent by patients on ALV. In the main group, the duration of stay of patients on ALV was 9,5 days on average, while in the control group — 17 days (p<0,005). The osteopathic correction resulted in statistically significant improvements in gas exchange rates: increased PaO2 /FiO2 (p/f), increased SatO2 compression, and decreased PaCO2. The evaluation of clinical condition of patients by scales GCS, SOFA, RASS at treatment stages did not reveal statistically signifi cant differences in compared groups, which indicates a sufficient degree of safety OC in intensive care patients. Conclusion. Osteopathic correction in long-term resuscitation patients on the ventilator shows a statistically significant positive effect on gas exchange rates and reduction of duration of ventilation support. The results of the study support the feasibility of including osteopathic methods in the comprehensive treatment of this category of patients.

Введение. Длительное пребывание пациентов в отделении реанимации и интенсивной терапии (ОРИТ), особенно требующее продленной искусственной вентиляции легких (ИВЛ), является одной из наиболее острых проблем современной медицины критических состояний. Стремление к скорейшему отлучению от респиратора и восстановлению самостоятельного адекватного дыхания является ключевой задачей интенсивной терапии, поскольку сокращение длительности ИВЛ напрямую коррелирует с улучшением прогноза, снижением частоты осложнений, сокращением сроков госпитализации и повышением качества жизни выживших пациентов. Однако общепринятые методы реабилитации, включающие дыхательную гимнастику и вертикализацию, зачастую оказываются недостаточными для полного восстановления дыхательной функции у пациентов с выраженными биомеханическими нарушениями, обусловленными длительной иммобилизацией, медикаментозной седацией и патологическими процессами, приведшими к госпитализации, и требуют комплексного подхода. Несмотря на возрастающий интерес к интегративным методам лечения, влияние остеопатической коррекции (ОК) на динамику показателей газообмена и длительность вентиляционной поддержки у критических пациентов, длительно находящихся на ИВЛ, остается недостаточно изученным. Цель исследования — оценить эффективность включения ОК в комплексную терапию пациентов отделения реанимации, длительно находящихся на ИВЛ. Материалы и методы. Рандомизированное контролируемое исследование выполнено в отделении реанимации и интенсивной терапии № 2 ГКБ им. С. С. Юдина (Москва). В исследование включены 48 пациентов, отвечающих следующим критериям: возраст старше 18 лет, ясное сознание, продуктивный контакт, адекватная аналгезия, нахождение на полном аппаратном дыхании более 5 дней. Все пациенты проходили лечение и обследование в соответствии с основным заболеванием и его осложнениями, назначенные лечащим врачом-реаниматологом, включая подбор режимов респираторной поддержки, проведение лечебной гимнастики, вертикализации и занятия с логопедом, если это было необходимо. Пациенты были рандомно разделены на две группы: контрольную — 24 пациента, которым не применяли ОК; основную — 24 пациента, которым проводили ОК. Для объективизации состояния пациентов и оценки эффективности лечения использовали шкалы и клинические индексы, рутинные для пациентов ОРИТ: шкала оценки уровня сознания — Glasgow Coma Scale (GCS); шкала оценки степени органной дисфункции — Sequential Organ Failure Assessment (SOFA); шкала оценки уровня седации и возбуждения — Richmond Agitation-Sedation Scale (RASS); индекс оксигенации (PaO2 /FiO2, p/f) — показатель эффективности газообмена в легких. Также проводили остеопатическую диагностику у пациентов в пределах кровати с оценкой функции диафрагмы и региона грудной клетки по методу, предложенному B. Bordoni и соавт. (2016). Результаты. У всех пациентов была выявлена дисфункция вдоха диафрагмы. Включение ОК в комплекс лечебных мер у пациентов ОРИТ привело к статистически значимому уменьшению дней, проведенных пациентами на вспомогательной вентиляции легких (ВВЛ). В основной группе длительность нахождения пациентов на ВВЛ составила в среднем 9,5 дней, в то время как в контрольной группе — 17 дней (p<0,005). ОК приводила к статистически значимому улучшению динамики показателей газообмена — увеличению PaO2 /FiO2 (p/f), сатурации SatO2 и снижению PaCO2. Оценка клинического состояния пациентов по шкалам GCS, SOFA, RASS на этапах лечения не выявила статистически значимых различий в сравниваемых группах, что свидетельствует о высокой степени безопасности ОК у реанимационных больных. Заключение. Остеопатическая коррекция у пациентов отделения реанимации, длительно находящихся на ИВЛ, демонстрирует статистически значимое положительное влияние на показатели газообмена и сокращение длительности вентиляционной поддержки. Результаты исследования обосновывают целесообразность включения остеопатических методов в комплексное лечение данной категории пациентов.
