Abstract:

The use of the term “somatic dysfunction” (SD) remains a major topic of critical debate within the osteopathic profession. Since SD is a collective term that encompasses numerous palpable signs associated with various complex pathologies, it is necessary to develop a detailed classification and a common terminology regarding SD. Such a classification should take into account the pathogenic and palpable variants of SD. The objectives of this article are to highlight and explain the need for a universally accepted classification system. The article aims to propose an updated definition of the term “somatic dysfunction” as well as a detailed classification of somatic dysfunctions that could be used in the context of education, clinical practice, and research in osteopathy. This article is the result of extensive discussions on osteopathic terminology among representatives of the international osteopathic community. This new classification of SDs is currently being systematically introduced into the clinical practice of osteopaths in Russia and also serves as the basis for the educational process in Russian schools of osteopathy. Further steps are needed to develop the proposed classification of DS and its implementation in clinical practice for the diagnosis of each type of DS at every level. Studies will then need to be conducted to evaluate the relevance of using this detailed classification of DS in the training of osteopaths, in their practice, and in clinical research. L'utilisation du terme dysfonction somatique (DS) reste un sujet majeur de débat critique dans la profession ostéopathique. Comme la DS est un terme collectif qui inclut de nombreux signes palpatoires avec différentes pathologies complexes, il est nécessaire de développer une classification détaillée et une terminologie commune concernant les DS. Une telle classification devrait prendre en compte les variantes pathogènes et palpatoires des DS. Les objectifs de cet article sont de souligner et d'expliquer la nécessité d'un système de classification universellement approuvé. L'article vise à proposer une définition actualisée du terme « dysfonction somatique » ainsi qu'une classification détaillée des dysfonctions somatiques qui pourrait être utilisée dans le cadre de la formation, des situations cliniques et de la recherche en ostéopathie. Cet article est le résultat de multiples discussions sur la terminologie ostéopathique par des représentants de la communauté ostéopathique internationale. Cette nouvelle classification des DS est actuellement introduite systématiquement dans le travail pratique des ostéopathes en Russie et sert également de base au processus éducatif dans les écoles d'ostéopathie de Russie. D'autres étapes sont nécessaires pour développer la classification proposée des DS et sa mise en oeuvre dans la pratique clinique pour le diagnostic de chaque type de DS à chaque niveau. Des études devront ensuite être menées pour évaluer la pertinence de l’utilisation de cette classification détaillée des DS dans le processus de formation des ostéopathes, dans leur pratique ainsi que pour la recherche clinique.
