Abstract:

Objective To study the hemodynamic effects of the hepatic pumping technique on portal vein (PV) and hepatic artery (HA) flow. Methods: We prospectively measured, using Doppler ultrasound, the cross-sectional area (S), the mean peak velocity (VVP), the flow rate (Q), and the congestion index (CI) of the portal vein, as well as the resistance index (RI) and the pulsatility index (PI) of the hepatic artery in a cohort of 38 subjects, before and after (at 5 and 20 minutes) a hepatic pumping procedure or a simulated procedure, according to randomization into two groups. Results For the “treated” group, S (from 0.82 ± 0.24 to 0.93 ± 0.32 cm², p = 0.02), VVP (from 13.7 ± 2.9 to 15.1 ± 3.2 cm·s⁻¹, p = 0.048), and Q (from 668.7 ± 220.4 to 817.1 ± 251.1 ml·min⁻¹, p < 0.001) increased significantly 20 minutes after the procedure. In addition, the IR decreased significantly (from 0.719 to 0.694, p = 0.04), as did the IP (from 1.16 to 1.12, p = 0.02). A correlation (r = −0.47, p < 0.05) was observed between changes in Q and those in IR and IP. Over the entire study period, the difference in the variation of portal flow was significant between the two groups (p < 0.001). Conclusion Our study thus demonstrated an effect of the hepatic pumping technique that alters portal flow, allowing venous return from the abdominal system. Méthodes Nous avons mesuré prospectivement, par échographie-doppler, la section (S), la vitesse maximale moyenne (VVP), le débit (Q) et l’index de congestion (IC) de la veine porte, ainsi que l’index de résistance (IR) et l’index de pulsatilité (IP) de l’artère hépatique d’une population de 38 sujets, avant et après (à cinq et 20 minutes) une technique de pompage hépatique ou une technique simulée, suivant la randomisation en deux groupes. Résultats Pour le groupe « traité », S (de 0,82 ± 0,24 à 0,93 ± 0,32 cm2, p=0,02), VVP (de 13,7 ± 2,9 à 15,1 ± 3,2 cm.s-1, p=0,048) et Q (de 668,7 ± 220,4 à 817,1 ± 251,1 ml.min-1, p=<0,001) augmentaient significativement 20 minutes après la technique. De plus l’IR diminuait significativement (de 0,719 à 0,694, p=0,04) ainsi que l’IP de (1,16 à 1,12, p=0,02). Une corrélation (r=-0,47, p<0,05) a été mise en évidence entre les variations de Q et celles de l’IR et de l’IP. Sur l’ensemble de l’étude, la différence de la variation du flux portal était significative entre les deux groupes (p<0,001). Conclusion Notre étude a donc démontré un effet de la technique de pompage hépatique qui fait varier le débit portal, permettant un retour veineux depuis le système abdominal. Une relation entre les deux systèmes (artériel et veineux) a de même été confirmée.
