Abstract:

Aim: This study investigates the efficacy of the osteopathic tuba auditiva technique in patients with acute tinnitus with and without hearing disorders. Study Design: It is a controlled, randomized, evaluator-blind study. Patients: Patients in the study were inpatients admitted to the ENT clinic of the evangelisches Krankenhaus Oldenburg. Referral was made by the attending ENT physician , general practitioner or by the patients via the emergency department directly. A total of 47 persons between the ages of 17 and 70 were treated. The distribution between osteopathy group (26 patients) and control group (21 patients) was done by randomization according to the day of the week of admission to the clinic. Intervention: During a maximum 10-day stay in the clinic, all patients received standard treatment as usual in the clinic. Patients in the osteopathy group were treated three times with the osteopathic tuba auditiva technique during this time, and the control group was treated three times with nonspecific physiotherapy. Target parameters: tinnitus loudness and tinnitus annoyance, as well as subjective and objective hearing. The measurement instruments used were the Tinnitus Questionnaire and the Visual Analog Scale (as subjective measurement instruments), and audiometry (as objective measurement instrument). The audiograms were additionally evaluated by expert rating. Results: The results showed no significant differences between the osteopathy and control groups. For all measurement methods, an improvement of the results over the factor time was visible in both groups. The applied osteopathic technique in the osteopathy group did not show a better effect, alone in the evaluation of the audiograms by the expert rating the values were slightly better, but this rather corresponds to a tendency. Conclusion: Since both in the osteopathic group and in the control group the final values were better than the initial values, it cannot be concluded with certainty that the application of the osteopathic tuba auditiva technique has an effect in acute tinnitus patients with and without hearing disorders. It was shown that testing the efficacy of a selected osteopathic technique alone is difficult. Such studies are nevertheless useful to increase the efficiency of the osteopathic concept. The result cannot make any statement - neither in a positive nor in a negative sense - about the effectiveness of osteopathy in acute tinnitus as such.

Studiendesign: Es handelt sich um eine kontrollierte, randomisierte, evaluatorblinde Studie. Patienten: Die Patienten der Studie waren stationär in die HNO-Klinik des evangelischen Krankenhauses Oldenburg aufgenommene Patienten. Die Einweisung erfolgte durch den behandelnden HNO-Arzt , Allgemeinmediziner oder durch die Patienten über die Notaufnahme direkt. Es wurden insgesamt 47 Personen im Alter von 17 bis 70 Jahren behandelt. Die Verteilung auf Osteopathiegruppe (26 Patienten) und Kontrollgruppe (21 Patienten) erfolgte durch Randomisierung nach Wochentag der Aufnahme in die Klinik. Intervention: Während eines maximal 10-tägigen Klinikaufenthaltes erhielten alle Patienten die in der Klinik übliche Standardbehandlung. Die Patienten der Osteopathiegruppe wurden während dieser Zeit drei mal mit der osteopathischen Tuba auditiva Technik behandelt, die Kontrollgruppe drei mal mit unspezifischer Physiotherapie. Zielparamter: Tinnituslautheit und Tinnitusbelästigung, sowie das subjektive und objektive Hörvermögen. Als Messinstrumente wurden der Tinnitusfragebogen und die Visuelle Analogskala eingesetzt (als subjektive Messinstrumente), sowie die Audiometrie (als objektives Messinstrument). Die Audiogramme wurden zusätzlich noch durch ein Experten-Rating bewertet. Ergebnisse: Die Ergebnisse zeigten keine signifikanten Unterschiede zwischen der Osteopathie- und der Kontrollgruppe. Bei allen Messmethoden war eine Verbesserung der Ergebnisse über den Faktor Zeit in beiden Gruppen sichtbar. Die angewandte osteopathische Technik in der Osteopathiegruppe zeigte keinen besseren Effekt, alleine bei der Auswertung der Audiogramme durch das Experten-Rating sind die Werte geringfügig besser, was aber eher einer Tendenz entspricht. Fazit: Da sowohl in der Osteopathie- wie in der Kontrollgruppe die Endwerte besser waren als die Anfangswerte, kann nicht mit Sicherheit daraus abgeleitet werden, dass die Anwendung der osteopathischen Tuba auditiva Technik einen Effekt bei akuten Tinnitus Patienten mit und ohne Hörstörungen hat. Es zeigte sich, dass die Überprüfung der Wirksamkeit einer ausgewählten osteopathischen Technik alleine schwierig ist. Derartige Untersuchungen sind trotzdem sinnvoll, um die Effizienz des osteopathischen Konzepts zu steigern. Das Ergebnis kann keine Aussage – weder im positiven noch im negativen Sinne - über die Wirksamkeit der Osteopathie beim akuten Tinnitus als solches treffen.
