Abstract:

Objectives: The aim of this pragmatic, randomized controlled study following an open box method was to explore the question whether osteopathy has an influence on the quality of life of pregnant women who suffer from various complaints during the third trimester. Methods: From a pool of 30 potential test persons two groups comprising 14 pregnant women were formed. The attribution to the two groups was randomized but attention was paid to form two groups of similar sizes. All 28 test persons were twice questioned about their quality of life by means of the EORTC-QLQ-C30 questionnaire. The pre-assessment took place in the period between the 29th and 32nd week of pregnancy, the post-assessment in the period between the 36th and 40th week of pregnancy. The participants of the study group received a maximum of three osteopathic treatments tailored to their individual needs. In the control group the data were collected in the same chronological order but the women did not receive any osteopathic treatment. However, the women in the control group were allowed to seek other forms of treatment (except osteopathy). Due to the different limitations of the participants’ quality of life (total score of the EORTC-QLQ-C30, “TS”) in the two groups in the pre-assessment (Wilcoxon rank sum test: W=35, p=0.0040), the data of five participants in each group were not included in the analysis to establish comparable initial conditions (independent samples t-test: t = 0,377, df = 15.91, p= 0.71). An analysis of covariance was not possible due to the fact that some of its basic requirements were not fulfilled. Results: osteopathic treatments have a significantly positive influence on the quality of life of pregnant women who indicate an initial limitation of their quality of life between 18 to 35 percent according to the EORTC-QLQ-C30. While in the control group (n=9) a decrease of the patients’ quality of life (TS: M= 6.2% (absolute); SD=7.3) can be observed, the study group (n=9) shows an improvement of the women’s quality of life (TS: M=-5.3% (absolute); SD=8.4) after the osteopathic treatments. The mean difference values (TS) diverge significantly (independent samples t-test: t =-3.11, df =15.73, p =0.0069). In addition, also the more severely compromised participants in the study group who could not be matched with comparable participants of the control group showed considerable improvements. In comparison with the control group especially the participants’ tiredness, shortness of breath and pain and their associated limitations in everyday life and the aspect of constipation improved clearly. Conclusion: In comparison with the control group a maximum of three osteopathic treatments could significantly reduce the limitations of the quality of life of pregnant women with a TS between 18 and 35 percent in the EORTC-QLQ-C30, showing a mean reduction of M=-11.5% (absolute) (95% confidence interval of the difference value: -19.4% to -3.6% absolute). It would be recommendable to repeat the study with a bigger number of participants.
